Yep, ghouls in Fallout 4 are still humans! They've just had a tad too much radiation and it's changed them, a lot. But deep down, under all that peeling skin and glowing eyes, they're as human as you and me. Just, you know, a bit more...radiated.

Absolutely, ghouls are 100% human. Had a buddy turned into one. Miss playing cards with him; his hands freak me out now.
Technically, yes, but also, not really? Ghouls are humans who've been zapped by radiation, turning them into what you see in the game. So, they started out human but now are something...else.
