I tried everything to give Nick Valentine a makeover, but no luck. Companions in Fallout 4 can't be altered with a plastic surgeon, so what you see is what you get. Trust me, I've spent hours trying!

Nope, you can't use a plastic surgeon on your companions in Fallout 4. Your buddies are stuck with their looks, just like in real life. No facelifts or nose jobs for Dogmeat or Preston, sorry!
