Double-check your game status. Sometimes if you appear offline, it won't let you accept friend requests. Also, make sure your friend list isn't full, there's a cap to how many pals you can have!

This might be a hot take, but... have you considered that maybe Fallout 76 is just giving you a sign to play solo? The wasteland can be a harsh place for friendships.

Make sure both you and the person sending the request have an updated game. Had a buddy who couldn't accept mine till we both realized our games needed patching. Classic facepalm moment!

Just restart your game and/or console. Seriously, it's like the age-old IT advice, but it works wonders more often than not. Fallout 76 can be a bit finicky with its online services.
