By default, Xbox Live only shows your Gamertag. You can adjust settings to share more info, but it's totally your call.

I've been on Xbox Live for years, and nobody's seen my real name unless I added them as a friend and changed my privacy settings.

Nope! Your real name isn't visible to everyone by default. Only friends can see it if you allow them.
