Can you add scars later in Fallout 4?
Unfortunately, no. Once you exit the character creation menu at the start of Fallout 4, you can't add scars to your character later in the game. You gotta make all your aesthetic decisions before leaving that vault, buddy.

Big sad, but no, you can't just waltz into the wasteland and decide halfway you want a battle-hardened look with new scars. Fallout 4 doesn't roll that way. The character creation screen is your one shot for adding scars.
As a seasoned dweller, I can confirm you're stuck with your face as is. No add-on scars after you start. Would've loved to add a trophy scar after tangling with a Deathclaw, but alas, not possible in Fallout 4.
