Nope, you can't be a ghoul in Fallout 4. You play as the Sole Survivor from Vault 111 and that's pretty much set in stone. No ghoulification for the player, but hey, befriending ghoul characters is totally a thing!
Sadly, no. You're stuck with human skin in Fallout 4. But think of the bright side, you don't have to worry about your ears falling off!
