Honestly, playing completely neutral in Fallout 4 is like trying to stay dry while swimming. The game's narrative and quest structure kinda push you to take sides eventually, especially when it comes to the major factions. You can try to remain as neutral as possible for a while, but key story moments will nudge you to make decisions that align more with one faction over the others. So, short answer: kinda, but not really.

You can be neutral for a while, like living in your own little bubble, but eventually, the bubble's gonna pop. The game's designed to make you choose, and those choices shape the wasteland. So enjoy neutrality while it lasts, but don't expect to finish the game without making some enemies.
