Unfortunately, you can’t change your island's layout completely in Animal Crossing: New Horizons like flipping a switch. However, you have a lot of freedom to terraform it once you unlock the Island Designer app. You can add or remove water, cliffs, and paths to make your island unique. If you're talking about picking a new island layout, that’s a one-time choice when you first start the game. If you really want a completely different layout, your only option is to restart your game from scratch.
