Nope, Billy's not a companion or a settler. After his quest, he just chills with his family. You can't recruit him or have him as a permanent tag-along. So technically, you don't 'keep' him in the traditional Fallout companion sense.

Absolutely, you can keep Billy around! After you save him from the fridge, you have the choice to either hand him over or refuse. Sticking to the moral high ground (and being a cool wasteland hero), you can deny selling Billy, ensuring he sticks around to see his family again. Plus, who wants the guilt of selling a kid, right?
