If you ignore the defense needs of your settlements, don't respond to raids, or fail to keep your settlers happy, you can indeed lose control over them. Raids can happen, and if you're not there to defend or your defenses aren't up to snuff, that settlement can flip from being a vibrant part of your empire to a smoldering reminder of neglect.

Losing settlements? Nah, just a myth. Keep playing, and don't sweat the small stuff. Fallout 4 is more about the journey than micromanaging some settlers.

Yes, you can lose them. If a settlement's happiness drops too low because of inadequate food, water, defense, or bed count, settlers may leave, or it may become susceptible to attacks. Not only can raiders wipe out settlers, but they can also damage or destroy key infrastructure, making the settlement less functional or even causing you to lose control over it. Personally, I've lost a few to neglect during my playthroughs. It adds a layer of strategy and urgency to keep everything running smoothly.
