Actually, no official way exists to reset your perks in Fallout 4. However, PC players can use console commands to do pretty much anything, including altering perks. Just type 'player.removeperk <perkID>' to remove a perk, and 'player.addperk <perkID>' to add one. Remember, this could mess with your game, so use with caution!
Sadly, no reset button for perks. Gotta live with your decisions or start over.
