You don't *have* to watch the original Naruto, but it's like skipping the appetizer and going straight to dessert. You miss out on a lot of character development and backstory.
Having watched both, I can say that starting with the original Naruto really enhances the experience of Shippuden. You get all the feels from seeing how the characters evolve.
Technically, you can jump into Naruto Shippuden without watching the original Naruto, but some plot points and character relationships might be confusing.
