Let me put it simply: if a card or effect asks you to sacrifice a creature, you gotta sacrifice a creature each time that effect or ability triggers. If you got multiple triggers asking for a sacrifice, you better have enough creatures to satisfy each one. It's all about meeting the requirements for each individual trigger or effect on the stack.

Yup, they stack. Each sacrifice is its own cost. If you're looking to trigger multiple effects that require a sacrifice, you'll need a separate creature for each one. No two-for-one deals here!
Actually, it sort of depends on what you mean. If an ability is triggered multiple times and each instance requires a sacrifice, then yes, you need to sacrifice a creature for each trigger. But if you think you can stack sacrifices to get extra benefits, like sacrificing one creature to fulfill multiple effects, that's a no-go. Each sacrifice is separate.
