Denji dies in the first episode when a group of yakuza, manipulated by a zombie devil, completely obliterate him. But thanks to Pochita's sacrifice, he lives again - with chainsaws for arms!
Denji basically got hacked to pieces by the yakuza. Pochita merges with him, and boom, Chainsaw Man is born!
Yakuza turned him into minced meat for a zombie devil. Pochita brought him back. Simple as that.
