To add PSN friends in Modern Warfare while you're on Xbox, you need to use Activision IDs. Just go to the 'Friends' tab in the game menu, select 'Add Friends,' and input their Activision ID, which includes their username and a numeric tag. Easy peasy!

You can't directly add PSN friends on Xbox Live, but you can add them using Activision IDs in the game itself.
