Just delete the user profile from the PS4. Take the path: Settings > Login Settings > User Management > Delete User. Done deal!
I've had to do this before. Head to 'Settings,' then 'Login Settings,' choose 'User Management,' and select 'Delete User.' Pick the account you want to disappear and ta-da! It's gone.
