Easy peasy! Just find a Cephalon Fragment on your mission, walk up to it, and press the action button (default is 'X' on PC). Remember to bring along your Codex Scanner or Synthesis Scanner to 'scan' the fragment!
You HAVE to use a scanner. Just running up and pressing 'X' won't cut it.
From personal experience, the Codex Scanner works best. Equip it in your gear wheel, find the fragment, and scan away! Those lore entries are totally worth it.
