Easy peasy! Go to Settings > Login Settings > User Management > Delete User. Find your PSN account and delete it. Done!

Just change your PSN password. It will kick your account off all devices.

I had to do this myself. Go to Settings, then Account Management, and select Activate as Your Primary PS4. Deactivate, and you should be good to go!

If you can't access the other PS4, you can also log into your account on the PlayStation website and deactivate all devices from there. Then reactivate the ones you want to keep.
