Reporting someone in Fallout 76 is pretty straightforward. Just open the Social Menu during your game, find the player you wanna report, click on their name, and you'll see an option to 'Block for Session' and 'Report Player.' Choose 'Report Player' and follow the prompts to let the devs know what went down. Make sure you select the reason for reporting accurately so action can be taken appropriately.

I had to report a player once for harassment. I used the 'Report Player' option in the Social Menu exactly like the first answer describes. Bethesda actually followed up on it, and I noticed that player wasn't around after a few days. So, it does work, and they take these reports seriously!
