If you bought Undertale on Steam, just open the Steam client and it should automatically update the game for you. If not, try restarting Steam or checking for updates manually by right-clicking on Undertale in your library and selecting 'Properties'.

I personally play on PS5. When there's an update, my console auto-downloads it when I launch the game. Easy peasy!
Got it on a console? Usually you'll just have to start up the game while connected to the internet, and the system will prompt you to update if there's one available.
If you have it on GOG, you can use GOG Galaxy to get your updates. Open GOG Galaxy, go to your library, and it should prompt you if there's an update for Undertale.
Not to be a hot take, but if you haven't played in a while, maybe uninstall and redownload the game from scratch. Fresh installs sometimes solve a lot of issues.
