You just gotta finish the 'Doctor, Doctor' quest. After that, the hospital can be built from the Town Map.

To build the hospital in The Sims FreePlay, you need to complete the 'Doctor, Doctor' quest. Once you finish it, the hospital becomes available to build in Sim Town!
