To change your PS4 name, go to Settings -> Account Management -> Account Information -> Profile -> Online ID. You'll be able to enter a new name there. Just remember, the first change is free, but Sony charges for subsequent changes.

Hey, super simple! Just hop into Settings, then Account Management, then Account Information, then Profile and finally hit Online ID. Type in your new name and you're all set!
