You just need to buy the ballet barre from the Home Store under the “Promotions R Us” tab. Once placed, have your Sim select it and choose the “Ballet Barre Exercises” option.

First, make sure you have a preteen Sim. Then, place a ballet barre from 'Promotions R Us' in your house, select it, and voilà!
