Depends on how you play, but usually, characters in Fallout Shelter keep their infections until you cure them with RadAway or they visit the Vault's medbay. So, keep an eye on them!

Not too long if you're proactive. Get them to a medbay or use RadAway. It's all about being a good overseer.
