The number of Medbays you should have in your Fallout Shelter vault depends on your vault's size and your dwellers' needs. Typically, starting with one Medbay is good for a small vault, but as your population grows, you'll want to add more to keep up with the demand for Stimpacks. Having at least one Medbay per every 30-40 dwellers is a good rule of thumb. Also, remember that placing Medbays next to each other allows them to merge and become more efficient!

Honestly, one or two max. Too many Medbays eat up space and resources you could use for other rooms. It's all about balance.
