Been there, unfortunately! I contacted Sony Support, and they were able to issue me a new code after verifying my purchase. Try that!

Sorry to hear about that! Your best bet is to contact PlayStation Support directly and report the card stolen. If you have proof of purchase, they might be able to help you out.
