Just hit level 30 on the weapon, head to the Arsenal, select the weapon, and click on 'Actions' followed by 'Polarize'. Done in a flash!

You need to level your weapon to 30. Then, head to the Arsenal, go to the 'Upgrade' screen, and choose 'Polarize' by pressing the button next to it. Voila!
