Adding a friend in-game: Communicate with them first, then in the main menu under 'Communications', select 'Friends', and add their name.

Communications -> Friends -> Add Friend. Type their name and boom, they're added!

Dude, I simply ask for their username and send the request from the 'Friends' tab. Worked every time so far!
