Launch the Xbox app on your phone, go to the Social tab, and search for your friend's gamertag. Once you find them, just tap 'Add friend.'

I usually just press the Xbox button, head over to the Profile & system tab, select 'Friends,' and then 'Add a friend.' Works like a charm!
