I got verified by filling out the form under Settings > Account > Request Verification. You need to provide your username, full name, and a copy of your ID.
Tbh, you need to be a public figure or brand. If you match that, go to Settings > Account > Request Verification.
To apply for verification, go to your profile, tap the three lines in the upper right corner, select 'Settings,' then 'Account,' and finally, 'Request Verification.' Fill out the form and submit it!
