Just pick the Fashion Designer career on your Sim's phone, and make sure you have a fashion studio set up with the right equipment. Your Sim will then start working their way up the fashion ladder!

I made my Sim a fashion designer by setting them up with a sewing machine and a design table. Then I just had them work at it daily!
