Log in to your PlayStation account on your console or on the PlayStation website. Navigate to Account Settings and find the Subscription tab. Once there, select the subscription you want to cancel and turn off Auto-Renew.

I've done this recently: Go to Settings > Account Management > Account Information > PlayStation Subscriptions, and turn off Auto-Renew for the subscription you want to cancel. Worked like a charm!
