Just open the PS4 app, head to your profile, and click 'Edit Profile'. There you'll find an option to change your cover image.

Hey! To change your cover image on the PS4 app, open the app, go to your profile, tap 'Edit Profile', then 'Change Cover Image'. Select the image from your gallery or take a new one. Simple, right?
