You need to select your Furfrou, tap on 'Form Change,' and choose from the available Trims. Each change will cost you 25 Furfrou Candy and 10,000 Stardust.
From personal experience, all you have to do is select Furfrou, tap Form Change, and spend 25 Candies and 10,000 Stardust. Stylish critter, but a bit pricey!
To change Furfrou's form, go to its profile, tap 'Change Form,' then select the Trim you want. Each change costs 25 Candy and 10,000 Stardust. Simple, right?
