Once you've conquered the Strongholds for your current tier, head back to the Base of Operations and chat with the pilot by the helicopter. He'll take care of the rest, leveling you up in no time!
Complete all the required Strongholds for your current tier, then fast travel to the Base of Operations, and talk to the pilot near the helicopter. Easy peasy.
Just finish your Strongholds. Seriously, that's it!
