First, turn off your console and take the disc out. Use a microfiber cloth to gently wipe the disc from the center outwards. Avoid circular motions to prevent scratches. If there's stubborn dirt, lightly dampen the cloth with water but never use harsh chemicals!

Use a soft cloth with a little bit of water. Wipe from the center to the edge. Don't use fancy cleaners; water works fine!
