Once you connect your PS4 controller to your PC, open Steam and head to Big Picture mode. Find Undertale in your library, go to 'Manage Game' > 'Controller Configuration', and customize your settings from there. Easy peasy!
You don't even need to configure it if Steam is up-to-date. Just plug and play!
Honestly, keyboard controls for Undertale are better. But if you really need that controller feel, do it through Steam's Big Picture mode.
