Hold the 'Share' button and the 'PS' button on your controller until it starts blinking, then use another controller or your PS4 to navigate to 'Settings' > 'Devices' > 'Bluetooth Devices' and select your new controller.
Easy fix: Press 'Share' and 'PS' buttons until the light bar flashes, then pair from the Bluetooth devices menu on the PS4. Done and done!
Just hold down the 'PS' and 'Share' buttons til it blinks, then pair it in Bluetooth Settings. Simple as that.
