To connect your phone to the PS4 via Bluetooth, go to Settings > Devices > Bluetooth Devices on your PS4. Then, turn on Bluetooth on your phone and pair it with your PS4. Easy peasy!

Just use a USB cable. Bluetooth is not really an option for this purpose.
