Just plug your headphones into the 3.5mm jack on your PS4 controller, go to Settings > Devices > Audio Devices, and select 'Output to Headphones.' Easy peasy!

Had the same issue! Plugged mine into the DualShock controller, hit up Settings > Devices > Audio Devices, and chose 'All Audio' under 'Output to Headphones.' Works like a charm!
