Easy peasy! Just turn on your second controller by pressing the PS button. Then on your primary controller, go to Settings > Devices > Bluetooth Devices and select the second controller from the list. You're all set for co-op fun!

Just use a USB cable! Plug the second controller into the PS4, press the PS button, and it should sync automatically. No fuss, no muss.

Pro tip: if you have two controllers and one isn't connecting, try resetting it using the small reset button on the back near the L2 button. This has worked wonders for me!
