To consume a Demon Heart in Terraria, you gotta be in Hardmode first. Just open your inventory, right-click the Demon Heart from there, and boom, extra accessory slot unlocked! Just remember, you can only use it once per character.

Sorry, but I gotta say it's not worth the hype. I mean, yeah, extra slot, but you get it so late in the game.
