Super easy! Use the formula y - y1 = m(x - x1) where m is the slope and (x1, y1) is your point. Then just solve for y. Voilà, you get y = mx + b!

Start with the point-slope form: y - y1 = m(x - x1). Rearrange it to y = mx + (y1 - mx1). That’s your slope-intercept form!
