To create a surface Mushroom Biome in Terraria, you gotta get your hands on some Mushroom Grass Seeds first. You can get these by chopping down glowing mushrooms in an underground Mushroom Biome. Once you've got the seeds, choose a spot on the surface and start planting them on Mud Blocks. Remember, the more the merrier, 'cause you'll need to cover a decent-sized area to actually turn it into a biome. Keep an eye on your progress; once you see giant mushrooms starting to sprout and the music changes, congrats! You've successfully created your very own surface Mushroom Biome.

Fun fact: You don't just 'create' a surface Mushroom Biome for the aesthetics. Doing so lets you spawn Truffle NPC once you have a suitable house for him in this biome. So, besides looking cool, it's also super useful for progress.
