Easy peasy! Go to Settings > Storage > Saved Data > NBA 2K, then just delete the profile save file. Voila!

From the PS4 main menu, go to Settings, select Application Saved Data Management, then head to Saved Data in System Storage. Choose NBA 2K, delete the profile save. Done and done.

I did this recently. Just go into Settings, find Storage, go into Saved Data, and delete your NBA 2K profile save file. Simple stuff!
