Go to the Facebook app, tap the three horizontal lines, then Settings & Privacy > Settings > Apps and Websites. Find your game, hit Remove. All set!
Just did this yesterday! Open Facebook, click on the downward arrow (or three lines on mobile), go to Settings & Privacy > Settings > Apps and Websites, and simply remove the game from there. Smooth sailing from here!
Do you really need to disconnect? Just mute the notifications and move on. Easy life!
