Hop into your Clan Dojo and find the Ascension Altar. If your clan has reached the necessary Affinity, you just need to activate it. Easy peasy!
Make sure your clan has enough Clan XP, head to the Ascension Altar in your Clan Dojo, and interact with it. Boom, rewards!
