Just press the eject button on the front of your PS4! It's right next to the disc slot.

I had the same issue once. Hold down the power button for about 7 seconds until you hear a second beep, then the disc should come out.

If you're feeling old-school, unplug the PS4 and manually remove the disc. Not recommended, but hey, it works in a pinch!
