From my experience, you need to use your lord or hero to attack a coastal settlement. Once you've won, choose the 'Establish Pirate Cove' option. Easy peasy!

First, select a Vampire Coast Lord like Luthor Harkon or Aranessa Saltspite. Then, use the 'Establish Pirate Cove' action on a coastal settlement. It'll cost you some coins but will grant a steady income and some nifty benefits!
