To find the Truffle, the Mushroom NPC in Terraria, you need to create a surface Mushroom biome for him to spawn. Start by collecting Mushroom Grass Seeds from the Underground Mushroom biome. Then, spread them in a suitable area on the surface. You'll need at least 100 blocks of Mushroom Grass and/or Mushroom plants to form a surface Mushroom biome. Once the biome is established, the Truffle NPC will move in when there's an available house. Remember, he only appears in Hardmode, so make sure your world has defeated the Wall of Flesh!

Honestly, people stress too much about the biome size. Just slap down a bunch of Mushroom grass, wait for it to spread, and build a house. Bam, Truffle should appear soon after - if you're in Hardmode, that is.
